Important Deadlines to note:
March 21 - $300 deposit due along with Application Form.
April 22 - $700 along with rest of forms/paperwork due to turn in.
May 20 - $700 remaining balance due.

Student/Adult Consent (2 pages)
Code of Conduct
Physician's Form with proof of required travel immunizations
Required Vaccinations/non-prescription medication form
Group Travel Medical Insurance
Notary
Adult Leader/Chaperone Responsibilities
Copy of Medical Insurance Card
Copy of Passport (passport must not expire within 6 months of travel dates).
